HAZOP stands for Hazard and Operability study. It is a structured, team-based technique that uses guide words to examine a process design node by node and find hazardous deviations and operability problems.

A HAZOP study is a systematic review of completed process and instrumentation diagrams in which a multi-disciplinary team applies standard guide words to each node to identify how the design could deviate from intent, what the causes and consequences would be, and whether the safeguards are adequate.

Guide words are standard prompts, No, More, Less, Reverse, As Well As, Part Of and Other Than, applied to each process parameter such as flow, pressure or temperature. The pairing generates a deviation, for example No Flow or More Pressure, which the team then analyses.

A HAZID is an early, concept-stage workshop that names the major hazard categories before detailed drawings exist. A HAZOP is a later, more detailed guide-word study run once the P&IDs are complete. Most projects use a HAZID first, then a HAZOP once engineering is advanced.

Run a HAZOP once P&IDs are complete and before commissioning a new plant, after any significant process modification, on a periodic revalidation cycle, when a regulator or lender requires it, for an existing plant that has never had one, and after an incident to check whether the deviation could recur.

The report contains the node-by-node worksheets with deviations, causes, consequences and existing safeguards, a scored and prioritised action register with owners, a risk matrix and dashboard, and a management debrief of the critical findings.

A HAZOP should be chaired by an independent, trained facilitator who is not tied to the design or to any equipment supplier, working with a multi-disciplinary team from process, operations, instrumentation and maintenance.
